I, , invoke the rights granted to me as a consumer under the Fair Credit Reporting Act, as amended, to direct Rental Research Services, Inc., 7525 Mitchell Road, Suite 301, Eden Prairie, Minnesota 55344, (800) 624-7422, to investigate, exchange data and furnish consumer reports and/or investigative consumer reports to MBG Property Management who owns or manages Park Avenue Apartments 2801 PARK AV S MINNEAPOLIS MN 55407 for the following purposes:

- In direct connection with my Application For Apartment Occupancy and the correlating extension of credit;

- the regular review of my credit capacity and general reputation during the entire length of my lease;

- the review of updated information at the time of any lease renewal or transfer of units;

- the results of any reinvestigation or dispute conducted as part of a consumer review; and

- the collection of any and all unpaid monies owed pursuant to the terms of the lease.

I understand that the foregoing information is supplied to MBG Property Management and Park Avenue Apartments to induce them to rent to me and is true and correct in all respects. This is a transaction which I have voluntarily initiated.

I authorize whatever credit investigation Rental Research Services, Inc., MBG Property Management and Park Avenue Apartments consider appropriate. This investigation may include, but is not limited to, the exchange of information from any source I listed on this Application For Apartment Occupancy, which may become part of an investigative consumer report, as more fully discussed below.
